Our unique VIP membership gives you exclusive access to lower cost services for those who are frequent visitors of 4 Elements.
This membership is a 6-month minimum commitment with subsequent renewals if desired that give a lower rate for services. It gives you the best of our expertise and knowledge, and allows us to even combine services if desired – who says you can’t have IV hydration therapy with acupuncture for stress?

Membership Agreement & Terms

4 Elements Vitality Institute Patron (VIP) Membership Agreement
This Membership Agreement (“Agreement”) is entered into between 4 Elements Direct Primary Care & Wellness Space (“Practice”) and the undersigned member (“Member”) by accepting the terms of service set forth below.
1. Membership Details
· Membership Name: VIP Membership
· Membership Fee: $30 per month
· Minimum Term: 6 months
· Non-Transferable: This membership is non-transferable. It is exclusively for the use of the undersigned Member and may not be assigned or transferred to any other individual or entity.
2. Membership Benefits
· As a VIP Member, you are entitled to access Member Rates for aesthetics and wellness services offered by 4 Elements Direct Primary Care & Wellness Space. Member Rates will be publicly displayed for eligible services. Member rates may change due to market costs but will be updated on the web site http://www.4elementsaesthetics.com. Membership Card must be presented in order to obtain Member Rates for services. If card is not presented, then the Service will be provided at the standard non-member rate.
3. Payment Terms
· Member agrees to pay a monthly membership fee of $30 for the duration of the minimum term, totaling $180 for the first six months.
· Payment will be automatically charged to the Member’s chosen payment method on a monthly basis through the third-party vendor Square.
· Membership fees are non-refundable.
4. Minimum Term
· This membership agreement has a minimum term of 6 months.
· Member acknowledges and agrees that this membership cannot be canceled or terminated during the minimum term, and all monthly fees for the minimum term must be paid in full.
5. Termination
· After the minimum term has been completed, the membership may be renewed for subsequent 6 months by informing the office to renew the membership.
· Practice reserves the right to terminate this membership at any time for reasons including but not limited to violations of Practice’s policies, disruptive behavior, or non-payment of fees.
6. Amendments
· Practice reserves the right to modify the terms and conditions of this Agreement, including membership fees, with prior notice to the Member. Any modifications will be effective after the completion of the Member’s minimum term.
7. Governing Law
· This Agreement is governed by and shall be construed in accordance with the laws of the State of Connecticut.
8. Entire Agreement
· This Agreement constitutes the entire agreement between the parties and supersedes all prior understandings, agreements, representations, and warranties.
By accepting the agreement, Member acknowledges that they have read and understood the terms and conditions of this VIP Membership Agreement and agree to be bound by its terms.
